CG PPHT 2021 Application form:

Candidates who are interested in the B. Pharma and D. Pharma courses under Chhattisgarh state have fill the application form and here is the process-
Visit the official website of CG PPHT and register yourself by giving the required data.
Now fill the application form by entering all the required data including personal, Professional and communication information.
Candidates who are likely to appear in more than one course, have to submit the separate application form for their respective courses.
Candidates have to upload their photograph, signature and thumb impression according to the prescribed specifications.
After filling and submitting the details successfully, candidates must take the print out of the form for use in future.

Process of Filling the Application Form:1-Registration and Form Filling:
Visit the official website that is www.cgvyapam.choice.gov.in
Click on the ‘Application Form’ tab.
You will get an application link for CG PPHT 2021.
Select your respective exam.
Then comes to the form filling which required personal details, Academic details and Communication details.
Personal details includes Candidate’s Name, Mother’s and Father’s Name, Gender, Category, Date of Birth, Nationality, Persons with disability, , Identification Type, Identification Number, Place of Birth, State, District, etc.
Academic details includes  all the information regarding 10th & 12th such as School Board of Class 12 or equivalent Name of School/College (passed/appearing), Address of School/College, Place of study, Year of passing, State where School/College is located, District where School/College is located, Qualifying Exam Code, Year of passing/Appearing class 12 or equivalent, Percentage of marks obtained in Class 10th if passed, Enter Roll Number of class 12.
communication details includes  their permanent as well as correspondence address, qualification, accupation and annual income of their Parents/Guardian and the email id and registered number are automatically displayed from registration details.
Then enter the security pin and submit all the details.
2- Uploading scanned documents:
After filling all the required information, the candidates need to upload the scanned copies of their photograph and signature.
The photograph should be 3.5 cm x 4.5 cm and Signature should be on the blank space below.
In another box put a clear left thumb impression of yours and Scan and upload the application form in JPEG format only. The file size should not be greater than 40 KB and less than 50 KB.
After filling all the details and uploading the scanned documents, check the data once again and click on "I agree" button.
3- Application Fee Payment:
Candidates can pay the application fee through Net banking/Debit/Credit Cards then only ICICI Bank.
In case candidates want to pay the fee offline. Carry the print out slip of the registration ID to any SBI branch and remit the amount.
Application Fee:
Candidates can pay the application fee through online net banking or by way of Bank challan.
